Report of Possible Sexual Misconduct at SKSM

If you believe there has been an incident of possible sexual misconduct against a SKSM student by another student, faculty or staff member, or visitor of SKSM, or by a student, faculty, staff member or visitor of a SKSM affiliated school or program, you are encouraged to report such conduct as soon as possible. In order for SKSM to reach out to the reporting party, verify receipt of this form, and offer any additional information and/or support, the reporter's name is needed. A reporting party can also choose to be anonymous, in which case SKSM’s ability to respond will be limited.

Anonymous Reporting:

When and if you choose to pursue any further action through SKSM with respect to the materials you submit, provide this code to the Title IX Coordinator and they will pull up your information. If you need to add additional information to your file at a later time, please use the same code name.

Note: Depending on the information received, SKSM’s ability to respond to an anonymous report may be limited. The Title IX Coordinator and/or Deputy Title IX Coordinator will review anonymous reports received by SKSM and determine whether an investigation and response is appropriate.
